Please note that a formal referral is not an absolute requirement because there is no CMS code for Medical Cannabis Evaluations, which means that the cost of a Certification Evaluation is still an out of pocket expense. However, we are happy to receive formal referrals to assist with your internal tracking of patient specialty care.

QUALIFYING CONDITIONS

Original conditions in 2000 (SB 862): Cancer, Glaucoma, HIV/AIDS, a chronic condition that causes cachexia, severe pain, severe nausea, severe muscle spasms (ie. MS and Crohn's Disease), or seizures (ie. epilepsy).

Added by the Legislature in 2015 (HB 321): PTSD.

Added by the Legislature in 2017 (HB 1488): Lupus, Epilepsy, Rheumatoid Arthritis, Multiple Sclerosis.

Added by Petition in 2017: ALS.
